A complainant who seeks to file a grievance (Equal Educational Opportunity or Equal Employment Opportunity) may obtain the complaint form below.  Following thorough and accurate completion of the form, the complainant should schedule an appointment with the Affirmative Action Officer, Mr. Steven Fiedeldey ([email protected]) at which time the grievance will be formally submitted.
